【摘要】 ：［目的］观察光动力疗法联合化疗对老年中晚期消化道肿瘤患者生活质量的影响。［方法］15例失去手术机会的老年中晚期消化道肿瘤患者（食管癌7例、结肠癌4例、直肠癌4例）进行内镜下光动力联合化疗治疗。于治疗前及治疗后第3、6、9、12月末进行随访，采用WHOQOLBREF量表进行生活质量调查。［结果］15例患者半年生存率73%（11/15），1年生存率40%（6/15）；中位生存期为9个月。（46.4：59.5，P<0.05）和心理领域（56.3：67.5,P<0.05）的生活质量，但对社会关系（P=0.192）和环境领域（P=0.742）的生活质量无明显影响。有1例出现皮肤光敏反应，副反应率为7%（1/15）；1例食管癌患者术后1周出现大出血，并发症率为7%（1/15）。［结论］光动力疗法联合化疗可改善老年消化道肿瘤患者的临床表现和生活质量，是适用于老年患者的一种相对安全有效的新方法。

Abstract：［Objective］To evaluate the efficacy of photodynamic therapy（PDT）combined with chemotherapy on the quality of life of elderly patients with advanced gastrointestinal carcinoma.［Methods］Fifteen patients underwent PDT combined with chemotherapy due to inoperable esophageal（7/15），colonic（4/15）and rectal（4/15）carcinoma.Treatment response of the tumors was determined by followup 3，6，9 and 12 months after PDT.Side effects were recorded.Survival data were obtained in all the patients.［Results］ The follow up period for the 15 patients was 0.5 to 16 months with a median time of 7.5 months.The survival rates of half a year and one year for the patients were 73%（11/15）and 40%（6/15）respectively.The therapy could improve the recent（3 months）quality of life of patients on PHYS（46.4∶59.5,P<0.05）and PSYCH（56.3∶67.5,P<0.05）domain，but had no effect on SOCIL（P=0.192）and ENVIR（P=0.742）domain.Cutaneous photosensitization was encountered in 1 patient corresponding to a sideeffect rate of 7%.Major bleeding occurred in 1 patient with esophageal carcinoma one week after PDT corresponding to a complication rate of 7%.［Conclusion］ PDT combined with chemotherapy appears to be a promising effective treatment for the middle/late staged elderly patients with gastrointestinal cancer.
