【摘要】 ［目的］观察金银花水煎液尿道冲洗在防治导尿管相关性尿路感染中的临床疗效。［方法］80例需留置导尿的男性患者随机分为四组：金银花水煎液尿道冲洗组(A组)、25%聚维酮碘（PVP）尿道冲洗组(B组)、生理盐水尿道冲洗组(C组)和未进行尿道冲洗组(D组)，每组20例。四组患者均在第3天、第7天进行尿液细菌培养，7d后拔除导尿管，进行导尿管细菌培养。［结果］A、B、C、D四组置管第3天时的尿培养阳性率分别为0%、5%、5%、30%，第7d时的尿培养阳性率分别为5%、20%、25%、70%，导尿管培养的阳性率为10%、30%、25%、70%，经Fisher确切概率法检验，留置导尿3d时，金银花水煎液尿道冲洗组的感染率较未进行尿道冲洗组低，差异有显著性（P<0.01）；而使用生理盐水和PVP进行尿道冲洗组均与未进行尿道冲洗组无显著性差异，三种不同冲洗液之间比较亦无显著性差异。留置导尿7d时，进行尿道冲洗的三组的菌尿阳性率均较未进行尿道冲洗组低，差异有显著性（P<0.01）；而三组间比较差异无显著性（P>0.01）。［结论］尿道冲洗可降低导尿管相关性尿路感染的发生率，而使用金银花水煎液尿道冲洗效果最佳。

Abstract： ［Objective］To observe the clinical efficacy of irrigating urethra with honeysuckle waterdecoction on preventing the catheterassociated urinary tract infections.［Methods］80 male cases were divided into four groups in random: intraurethral irrigation with honeysuckle waterdecoction group（group A),intraurethral irrigation with 20% PVP（group B),intraurethral irrigation with normal saline group（group C) and control group（group D).There were 20 cases in each group.Urine of all cases were required for bacterial cultures at the 3rd day and the 7th day.The urethral catheters were removed at the 7th day,and then catheter bacterial cultures were performed.［Result］The rates of urinary tract infection in group A,B,C and D were 0%,5%,5% and 30% at the 3rd day after indwelling,respectively.And at the 7th day,the positive rates of the urine cultures in group A,B,C and D were 5%,20%,25%,70%,the positive rates of the catheter bacterial cultures in group A,B,C and D were 10%，30%，25%，70% respectively.It had been proved by Fisher Exact Test that the infection rate in group A was significantly lower than that in group D（P<0.01) at the 3rd day,while there were no statistical significance between group B and group D,group C and group D,and a [1] [2] [3] 下一页 |
